Stage 05 · the claim

Charge entry and claim scrubbing before anything is submitted

Charges reconciled to the day's schedule, then run through payer edits. A claim that goes out clean gets paid first time.

Clean claims are the benchmark

A clean claim rate of 95% or higher is the target. Under 90% signals a front-end problem — eligibility, registration or coding — that scrubbing alone cannot fix. That is why we run the whole cycle rather than just the submission step.

What happens before submission

  • Charges reconciled to the day's schedule, so no visit goes unbilled
  • Charge lag tracked from date of service to submission
  • Claims run through clearinghouse and payer-specific edits
  • Rejections corrected and resubmitted the same day
  • Submission confirmed through to payer acknowledgment

Working in your software

We work inside your practice management system and your clearinghouse. We don't sell an EHR, so we have no reason to move you onto one, and the licence stays in your name.

Charge lag and unbilled encounters

Charge lag is the time between the date of service and the date the claim goes out. Every day of lag is a day added to days in AR, and an encounter that is never billed is revenue that is never collected. We reconcile charges to the day's schedule, so a visit with no charge — an unsigned note, a missing superbill, a no-show marked as seen — is flagged and chased rather than discovered at month end.
