Small errors, expensive denials
A misspelled name, a transposed member ID or the wrong subscriber date of birth will each send a clean, correctly coded claim straight back. CO-16 — missing information — is the result, and it is common wherever registration happens fast: urgent care walk-ins leave place-of-service errors behind, and busy front desks skip fields under pressure.
What intake QA covers
We review registrations against the eligibility response before the claim is built.
- Patient name, date of birth and address against the payer record
- Member ID, group number and subscriber details
- Primary and secondary insurance order
- Referring provider and place of service
- Missing signatures, consents and patient forms
Feeding fixes upstream
When the same registration error keeps recurring, we say so — by location and by staff workflow — so it is fixed at the source. If you use our front office and scheduling coverage, intake is handled by the same named person who answers your phones.
Why it is checked before the claim is built
Registration errors are invisible until a payer reads them. By then the claim has been coded, scrubbed and submitted, and correcting it means a resubmission and another wait. Checking the registration against the eligibility response before the claim is built catches the mismatch while the fix is a ten-second edit — and the payer never sees the error at all.
