The measures we report
- Clean claim rate and first-pass resolution — target 95%+
- Days in AR and aging past 90 — target under 40 days
- Denial rate and top reasons by payer — target under 5%
- Net collection against contracted rates — target 96%+
- Charge lag and unbilled encounters
Why these four numbers
Under 90% clean claims signals a front-end problem. Past 50 days in AR, cash is sitting in unworked claims. A double-digit denial rate means the same mistakes keep repeating. And net collection measured against what your contracts entitle you to — not what you charged — is the only honest measure of whether you are being paid in full.
When to judge us
First-pass acceptance in weeks four to six shows whether the front end is fixed. By day 90, net collection, days in AR and denial rate reflect a full cycle with us. Reports come with a fortnightly status call with your named account lead.
Broken down so you can act on it
A practice-wide average hides the problem. Every measure is broken down by provider, location and payer, so you can see that one payer underpays a code, one location's registrations fail more often, or one provider's notes keep triggering documentation queries — and each finding comes with the step we are taking about it.
